New program focuses on enhancing centers’ rehabilitation programs

“Homeward Bound”
comes to Revera Health Systems

 

WAYNE, NJ, SEPT. 7, 2005 –

As comfortable and welcoming as our centers are, most patients hope to go home as soon as they can. That’s not just human nature–at Revera Health Systems it’s also a very realistic goal! And Revera Health Systems plans to acknowledge this goal by introducing a new program that strengthens our performance in short- term/rehab care.

The importance of rehab programs

It’s a fact: at most Revera centers, well over 90% of our rehab patients go home within 60 days of admission–and in many centers the percentage is over 95%. That’s an impressive success rate, and proves how effective our rehab programs really are.

We all know how much it means to residents and their families when patients complete a successful rehab program and are able to go home. But having an effective rehab program is also good for our centers because rehab patients are an important part of our resident mix. The better we are able to serve the needs of these patients, the more successful our centers will be.

In order to support and encourage our centers to attract and care for rehab patients, Revera is in the process of creating marketing materials aimed specifically at referral sources and potential users of our rehab services. The company is also introducing an internal incentive program called “Homeward Bound.”

Introducing “Homeward Bound”

What is “Homeward Bound” and how can it help your center increase enrollment of rehab patients? The core of the “Homeward Bound” program is an annual certification process that will designate and reward all Revera centers that meet the following criteria:

1) Over 95% of the center’s rehab patients must be discharged to home within 60 days of their admission.

2) Each rehab patient must be discharged with a clearly written discharge plan including confirmed referrals and follow-up appointments with appropriate healthcare providers.

3) Specific, scheduled follow-ups and progress reports will be made and confirmed.

4) Each center will hold a “rehab reunion” to maintain contact with former patients and support their ongoing improvement.
